Before your initial visit, you will receive an online intake form that will help your provider understand your health history and current struggles. You can submit your previous medical records to your provider too. During your initial visit, your practitioner will listen to your needs, evaluate all your symptoms, and provide a diagnosis. If appropriate, your provider will also go over your treatment plan during the initial visit.
