Make sure you have registered for the patient portal! Here's a step-by-step guide on how to book your follow-up appointment:
KiwiHealth EHR:
You may login directly here: https://my.helloklarity.com
1. Log into your KiwiHealth Client Portal and click the Appointments tab on the left-side menu.
Here you'll see any upcoming and past appointments. In the upper righthand corner you can click the blue "Book Appointment" button to get started with booking a new appointment with your provider.
2. Select type of appointment you'd like to book with your healthcare provider by checking off the desired appointment type. Click "Confirm Appointment Type" at the bottom of the page to move to the next step.
Your provider may offer multiple contact options, which you can select from, as you book your appointment:
- Video Call
- In-person
- Phone call
3. Choose your date and time from the availability offered.
4. Confirm your appointment.
You'll see a confirmation screen, and also receive a confirmation email. For your convenience, you can also add your appointment directly to your personal calendars (iCal, Google Calendar, and Outlook) by clicking the link in the confirmation screen or email.
IntakeQ EHR:
Step 1: Log In
Visit the client portal
Enter your credentials to access your IntakeQ Portal account
Step 2: Book Appointment
Once logged in, you'll find the "Book" tab at the top right of your screen. Click on it to get started.
Step 3: Select Your Location
Choose the state in which you currently reside. This helps us ensure you are directed to the correct provider.
Step 4: Choose Your Provider
Select your established healthcare provider from the list provided. This ensures you schedule your appointment with your current specialist.
**Please do not schedule an appointment with another provider. Only select your current provider. If you wish to be scheduled with another practitioner, please email [email protected] to initiate this process**
Step 5: Follow-Up Service
Choose the "Follow-Up Service" option. This will guide you to the specific follow-up appointment options available.
Step 6: Pick a Date and Time
Based on the designated time frame your provider recommended for your follow-up, select a date and time that best fits your schedule. The available slots will be displayed for you to choose from.
Step 7: Confirm Your Appointment
Review the details of your appointment, including the date and time. Confirm your booking.
That's it! Now you can successfully schedule your follow-up visit with your established provider. You'll receive a confirmation email with all the necessary information. Please make sure to complete your intake form before the scheduled appointment to avoid cancelation or additional fees.
