Step 3: Select Your Location
Choose the state in which you currently reside. This helps us ensure you are directed to the correct provider.
Step 4: Choose Your Provider
Select your established healthcare provider from the list provided. This ensures you schedule your appointment with your current specialist.
**Please do not schedule an appointment with another provider. Only select your current provider. If you wish to be scheduled with another practitioner, please email email@example.com to initiate this process**
Step 5: Follow-Up Service
Choose the "Follow-Up Service" option. This will guide you to the specific follow-up appointment options available.
Step 6: Pick a Date and Time
Based on the designated time frame your provider recommended for your follow-up, select a date and time that best fits your schedule. The available slots will be displayed for you to choose from.
Step 7: Confirm Your Appointment
Review the details of your appointment, including the date and time. Confirm your booking.
