How to Request a Prescription Transfer:

  1. Call Your Current Pharmacy – Contact the pharmacy that currently has your prescription and ask if they can transfer it.
  2. Provide New Pharmacy Details – Have the name, address, and phone number of the new pharmacy ready when making the request.
  3. Check for Restrictions – Some prescriptions, like controlled substances, may not be eligible for transfer. Your current pharmacy can confirm.

Important Information About Controlled Substances:

  • Practitioners cannot increase supply, reorder, or transfer controlled substance medications due to nationwide regulations.
  • If you need a refill or an alternative option, please work with your provider to determine the best course of action.

If Your Pharmacy Cannot Transfer the Prescription:

  • Message Your Provider in the Patient Portal – Include the name, address, and phone number of your new pharmacy so they can assist with sending a new prescription.
  • Ask Your Pharmacy to Order the Medication – If your medication is out of stock, your pharmacy may be able to order it instead of transferring.
