How to Submit a Prior Authorization Request
To initiate a prior authorization request, you can:
- Submit the request with your healthcare provider through the patient portal.
- Email our customer support team for assistance.
When starting a PA request, ensure your insurance information is uploaded to the portal. You will need:
- Insurance ID Number
- RxBIN#
- RxPCN#
- RxGroup#
- Photo of the front and back of your insurance card
Important Considerations
- Verification: Call your insurance company before your appointment to verify which medications are covered.
- Approval: Submitting a PA request does not guarantee that your health insurance will cover the cost of your medication(s). If denied, you may need to pay out-of-pocket or find alternative medications covered by your insurance.
