If you are planning to move to another state, our customer support admin team would like to help make the transition as smooth as possible. We recommend to always communicate with your provider via messages in the patient portal. Letting them know of your moving dates will be crucial to making sure the transition goes well. Appointments with a new provider will need to be set up to continue with a treatment plan.
You can reach out to the customer support admin team to inform us which state you are moving to. We will check if there are available appointments with a provider in that state. If so, we will let you know and send you a medical release form for signature (if applicable). After that is signed, a provider will review your medical records for a consideration of treatment. If accepted to their practice, we will help you book an initial appointment, which will be discounted to the follow-up pricing ($59).
Please note you will need to fill out a new initial intake form and the new provider cannot prescribe medication prior to having that first visit. They also cannot fix any medication issues from another provider, only the prescribing practitioner can do that.
If there are no available providers, we will send a blank medical release request. You will need to complete the form and let us know when it is finished. Then, we'll release your records to the secure patient portal after this has been received or they can be faxed to your new in=person provider.
